Doing Business Under Assumed Name Certificates

A sole proprietorship or general partnership that intends to use a name other than the owner’s (or owners’) personal name must file for a fictitious name through a process that is called “Doing Business As” (DBA). You are required to file in the county or counties in which you conduct business.

You may file in Carroll County by completing the Certificate of DBA form. There is a $25.00 filing fee. The County Clerk’s office accepts Visa, Mastercard, Discover (3% charge), check, money order, cashier’s check, or cash.

• IN PERSON: Please bring a valid photo ID of owner or owners and the filled-in, notarized form along with the $25 filing fee. We do have notaries on staff that can notarize the owner(s) signatures. Owner(s) must be present and sign in front of the notary. We will file the form and the original will be returned to the owner(s) usually within a few minutes.
• BY MAIL: The Certificate Form MUST be notarized prior to mailing and the $25.00 filing fee included. If either are not done, the document cannot be filed and will be returned. Be sure to indicate where the form will need to be returned to after filing along with a self-addressed stamped envelope.
